Area residents have access to GBFHT's services by getting a referral from their GBFHT Family Physician (only patients of affiliated physicians may be referred) * family physician referral also required for most clinics * GBFHT is the regional Diabetes Education Centre (DEC) * All patients with diabetes regardless of who their family doctor is may be seen at the DEC

Counselling: Individual and marital counselling available, including child and youth mental health counsellors for short-term mental healthcare * Patients 16 years and under can be helped with anxiety, depression, anger, acting out, conflicts with parents/siblings, addictions, as well as problems with school and socialization * Offers one-on-one or family sessions * Single session counselling available without appointment on first come, first served basis

Falls Prevention: The family health team assists seniors with preventing falls or recovering from falls by providing: * fall risk assessment/screening at annual physicals including basic falls prevention advice * after a fall, identifying the cause of the fall and helping patients develop strategies to prevent future falls * referral to other falls prevention programs or services * regular medication reviews * basic vision assessments and reminders at annual physicals to have regular biannual eye exams 
 
The Georgian Bay Family Health Team is a member of the Staying Independent Falls Prevention Coalition (SIFPC).

The Seniors Assessment & Support Outreach (SASOT) is an interdisciplinary team that consists of two Social Workers, an Occupational Therapist and a Registered Nurse. A physician performs the role of Medical Advisor.  
Patients may be seen in their own home, a retirement residence, or in the emergency department at South Muskoka Memorial Hospital.  
SASOT completes a comprehensive assessment of a patient's health and day-to-day function, including:  
* Safety/Home Environment  
* Medical Concerns  
* Memory/Cognition  
* Mobility  
* Personal Care  
* Household Activities  
* Social Issues  
* Mood  
 
The team will also assess patients in the hospital who are designated Alternate Level of Care and support them through the discharge process.

Map RecordOrillia Soldiers' Memorial Hospital, Orillia - Colborne St, North Simcoe Muskoka Integrated Regional Falls Program 705-330-3200 The program is a community based falls prevention program which is working towards a standardized regional approach to falls screening and assessment within the North Simcoe Muskoka area. 
 
The program works within local hospital emergency departments, providing community screening clinics and specialized assessment clinics with the goal of keeping seniors safe by reducing the risk of falls and falls related injuries.  
 
Clients include those who have had a fall or have a fear of falling.  
Clients are provided the following: 
* falls risk assessment 
* community resource information and specific recommendations to clients and their family physicians as required 
* for clients who have been seen for a fall in an emergency department in the region, there is emergency department support nurses available who make contact with the client to provide falls risk evaluation and support, which may include a home assessment  
 
Also, offer Community Falls screening clinics within the region. At these clinics, clients are individually seen by a registered nurse and/or physiotherapist. Program also has a staff clinical pharmacist who provides medication consultation.  
 
For clients with more complicated medical history the program also runs a specialized assessment clinic where referrals to geriatricians, physiotherapists and occupational therapists are possible.

* Product advice and help with product use, including assessments for assistive devices
