The Seniors Assessment & Support Outreach (SASOT) is an interdisciplinary team that consists of two Social Workers, an Occupational Therapist and a Registered Nurse. A physician performs the role of Medical Advisor.
Patients may be seen in their own home, a retirement residence, or in the emergency department at South Muskoka Memorial Hospital.
SASOT completes a comprehensive assessment of a patient's health and day-to-day function, including:
* Safety/Home Environment
* Medical Concerns
* Personal Care
* Household Activities
* Social Issues
The team will also assess patients in the hospital who are designated Alternate Level of Care and support them through the discharge process.

|Orillia Soldiers' Memorial Hospital, Orillia - Colborne St, North Simcoe Muskoka Integrated Regional Falls Program||705-330-3200||The program is a community based falls prevention program which is working towards a standardized regional approach to falls screening and assessment within the North Simcoe Muskoka area.
The program works within local hospital emergency departments, providing community screening clinics and specialized assessment clinics with the goal of keeping seniors safe by reducing the risk of falls and falls related injuries.
Clients include those who have had a fall or have a fear of falling.
Clients are provided the following:
* falls risk assessment
* community resource information and specific recommendations to clients and their family physicians as required
* for clients who have been seen for a fall in an emergency department in the region, there is emergency department support nurses available who make contact with the client to provide falls risk evaluation and support, which may include a home assessment
Also, offer Community Falls screening clinics within the region. At these clinics, clients are individually seen by a registered nurse and/or physiotherapist. Program also has a staff clinical pharmacist who provides medication consultation.
For clients with more complicated medical history the program also runs a specialized assessment clinic where referrals to geriatricians, physiotherapists and occupational therapists are possible.
